SARAH: Welcome back to the QI Hub video series. We are joined again by our favorite Dr. Sarah Adkins, clinical pharmacist. So thank you so much for being here. Dr. Adkins, or Sarah. DR. ADKINS: My pleasure. SARAH: Fantastic. Well, last time Dr. Adkins, Sarah, spoke with us about SGLT twos and the ADA guidelines, the standards of care. We talked about section nine, which is the pharmacologic approach, and last time she talked about SGLT twos. Today we're actually gonna talk about GLPs. I think it's a pretty hot topic, so we're gonna ask Dr. Adkins, Sarah, here to talk to us, gimme some information about GLPs. DR. ADKINS: Ooh. Fantastic. Thank you so much for that lovely introduction. And yes, today we are gonna talk about GLPs. Oh, GLPs. SARAH: Right? DR. ADKINS & SARAH: Right, where to begin. SARAH: There's so much. DR. ADKINS: It's a lot. Okay. So what we're gonna start with today all on GLPs, is we are gonna start with just talking briefly about maybe three of them. Which are the ones we hear the most about it. Okay. Alright. So the ones we're gonna talk about are a drug called tirzepatide. Which is the chemical name. A drug called semaglutide, chemical name. All right. And a drug called liraglutide. And my docs out there, I'm sure they know all these. Um, we'll just give a quick summary just for completeness sake. SARAH: Yeah. Tell us something we don't know. DR. ADKINS: Alright. So tirzepatide is actually available in two different brand names. The first one is Mounjaro. And that actually is, uh, FDA approved for type two diabetes. I know, right? Then the other one is actually Zepbound, and Zepbound, also tirzepatide is actually approved for weight loss. Okay, two different FDA approvals. Alright, the next one is Ozempic. Ozempic is semaglutide and it is approved for type two diabetes. And um, Wegovy is also semaglutide, but it is approved for weight loss. Right, see how this works. Are you picking up on this? SARAH: I got you. DR. ADKINS: Interesting, right. SARAH: I got you. DR. ADKINS: The last one I'm gonna talk about, only because this is the drug that's available through Rising Sun's Pharmacy. Is a drug called Trulicity. And that one is not, it doesn't, hasn't quite shown the weight loss benefits. Um, but just so you know that it, it's available there and the generic name for that is Dulaglutide. So, so that's there. It's available. Um, I want to say that the interesting thing about these is navigating formularies on these. Right. So people dunno what their insurance covers. That they don't know which one's approved, which one's not approved. I really encourage you and um, you and your patients to look at drug formularies to determine what's covered. Before you decide that it's just not a covered drug at all. Um, it may take some research and it may take some work on the patient's part to call their insurance and ask which drug is covered. But it's an important thing to do. Another weird thing I wanted to mention is how these are available. So the Mounjaro actually has a delivery device, that has a needle included and you can't see the needle. Which is actually kind of cool. SARAH: That is cool. DR. ADKINS: The Ozempic you actually, it comes with needles. So you actually put a needle on the top and then dial the dose. Okay. Kind of cool. And then the Trulicity is similar to the Mounjaro 'cause it's made by the same company. The same delivery device. So it's actually very easy to use, hides the needle. So it's actually kind of cool. Another one thing I wanted to say too, that I think is an interesting comment that I don't think people have really heard is that, um, that, that you have to be on this for the rest of your life. I do, I had somebody say the other day, they're like, "oh, I have to be on it for the rest of my life". The rest of your life is a long time SARAH: It's a long time DR. ADKINS: And I don't, I don't know if I believe that. So I mean, as we said earlier, I'm old, right. I've been in the practice for 30 years. Thanks for that. So what I'm saying is the rest of your life is a long time. So I want to just examine that statement and say, I dunno, I don't know if I believe that. So be careful what you, um, spread about SGLT twos, I guess. Oh no. Wrong drug. GLP, it's your fault. Alright. So that's what I wanted to share about formularies of GLPs. SARAH: Alright. Okay. So thank you for that. Thank you so much for sharing that formulary information. It's not my fault about the SGLT twos versus GLPs, just so you know that. Yeah, that's right. But what I would like to know is talk to us about compounding. There are a lot of compounded GLPs out there and I think it's a new thing. What's happening with that? I don't really understand what's happening. DR. ADKINS: Uh, okay. So compounded GLPs, when we talk about compounded GLPs, this is a thing, right? You see it advertised on television, you see it everywhere. What I would like to say as a pharmacist, to my providers, to my patients, just be very vigilant on where you are getting that medication. So when you can, when places compound a GLP, they can use the same ingredient, but it has to be maybe in a different formulation or delivered a different way. So there are examples of this online. So if you look it up, you can see that there are different people that are compounding this. So I would just encourage one to be, um, just, just use discretion in which, uh, compounded GLP you are seeking and who is providing that. So who's manufacturing it? And just ask questions around it to make sure that that keeps everyone safe. Um, that is my one caution with the compounded GLPs. Um, the interesting thing is it's, it's the way the FDA approval goes with the drug, they can sort of get away with redoing that compounding. So, um, I just wanted to mention that. SARAH: Interesting. DR. ADKINS: I think it's a, a hot topic, so if you wanna discuss more about that, you know, you can always reach out to the old pharmacist for a discussion. Thanks.
