Partnering with Healthcare Providers to improve diabetes outcomes
for the people of Southeastern Ohio.
Dr. Fitzgerald, Athens
"I love the Diabetes Navigation project. It gives me such peace of mind knowing I am referring my patients to someone who will take care of them - treat them like a person and connect them with valuable life changing resources."
Jill, Patient
"Utilizing real-time data from my continuous glucose monitor helps me make informed decisions about meals and activity, giving me greater control over my blood glucose levels."
Sarah, QI Lead
"It's incredible that we've increased prescribing of Continuous Glucose Monitors from 8% to 22% for Type 2 diabetes patients at our partner sites during the first year of the project."
What we do
The Southeastern Ohio Regional QI Hub is proud to partner with healthcare providers, community organizations, and patients to combat high rates of diabetes in Southeastern Ohio.
How we do it
Through a systematic, data-driven approach, we use quality improvement to enhance health outcomes and promote equity at our clinical sites. We also provide regionally tailored educational materials to share best practices in diabetes care with providers and the community.
Why we do it
In Southeastern Ohio, 1 in 5 adults has diabetes, and that climbs to over 1 in 3 when factoring in pre-diabetes and cardiometabolic conditions. This issue impacts us all, but we're ready to tackle it head-on.
